What You’ll Do:
  •  Provide 1:1 therapy sessions in patients’ homes.
  •  Evaluate and treat a primarily geriatric population.
  •  Create and implement individualized care plans.
  •  Maintain clear and timely documentation.
  •  Collaborate with a supportive clinical team.

Qualifications:
  •  Licensed OT in the state of NY
  •  Degree in Physical Therapy
  •  Strong communication and clinical skills 
  •  Home health experience (preferred, not required) 
  •  Reliable transportation
