Occupational Therapist (OT) – Home Health | Buffalo, NY | PRN & Part-Time | North & South Buffalo Opportunities

Stern at Home Therapy is hiring Occupational Therapists (OTs) for two flexible openings serving patients throughout the Buffalo, NY area.

We currently have opportunities covering both the North Buffalo and South Buffalo regions, allowing you to build a caseload close to where you live. Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.

Treat one patient at a time, manage your own schedule, and provide individualized care without the double-booking, productivity quotas, and clinic burnout common in traditional outpatient settings.

What You'll Do:
As an Occupational Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout either the North or South Buffalo service area.

Responsibilities include:
  • Perform occupational therapy evaluations and develop individualized plans of care.
  • Provide one-on-one treatments based on each patient's functional needs and goals.
  • Manage individualized treatment plans with clinical autonomy.
  • Help patients improve independence, safety, mobility, and their ability to perform daily activities.
  • Complete, accurate, and timely clinical documentation.
  • Communicate with patients, families, physicians, and members of the clinical team.
  • Coordinate with our administrative team regarding scheduling, patient care, and logistics.
  • Build a caseload based on your preferred geographic area and availability.

Compensation:

Part-Time W-2:
  • $65 per follow-up visit.
  • $75 per 4-unit evaluation.

PRN:
  • $62 per 4-unit follow-up visit.
  • $75 per evaluation.

Qualifications:
  • An active Occupational Therapist (OT) license in New York is required.
  • Reliable transportation required.

  • Ability to work independently in a home-based setting.
  • Home health experience is preferred but not required.
