Occupational Therapist (OT) – Home Health | Toms River, NJ | Part-Time or PRN

Stern at Home Therapy is hiring a part-time or PRN Occupational Therapist (OT) to provide one-on-one care to patients in their homes throughout Toms River and surrounding Ocean County communities.

This position offers flexible scheduling, clinical autonomy, and dedicated administrative support without the double-booking, clinic politics, or productivity pressure often found in traditional therapy settings.

What the Job Looks Like:
You will evaluate and treat a primarily geriatric patient population in their homes, creating individualized treatment plans based on each patient’s functional needs and goals.

You’ll have the autonomy to manage your own caseload and schedule while our administrative team handles referrals, scheduling coordination, authorizations, and other logistics.

Responsibilities:
  • Perform occupational therapy evaluations and develop individualized plans of care.
  • Provide one-on-one occupational therapy treatments in patients’ homes.
  • Help patients improve independence with activities of daily living.
  • Address upper-extremity function, strength, coordination, cognition, safety, and functional mobility.
  • Recommend adaptive equipment and home modifications when appropriate.
  • Complete accurate and timely clinical documentation.
  • Communicate with physicians, caregivers, family members, and the interdisciplinary care team.
  • Maintain compliance with applicable clinical and regulatory requirements.

Compensation:
Part-Time W-2:
  • $65 per follow-up visit.
  • $75 per four-unit evaluation.
PRN
  • $72 per follow-up visit.
  • $85 per evaluation.

Part-Time Benefits:

Qualifications
  • Active Occupational Therapist license in New Jersey.
  • Graduation from an accredited Occupational Therapy program.

  • Home health or geriatric experience is preferred but not required.
