Stern at Home Therapy is hiring a PRN Occupational Therapist (OT) to provide one-on-one therapy services at an established facility in Greece, NY.
This is a facility-based 1099 PRN opportunity ideal for an Occupational Therapist looking to supplement an existing schedule without the travel requirements of traditional home health.
All patients are treated at one facility, allowing you to maintain a convenient and consistent caseload without driving between patient homes.
What You'll Do
As an Occupational Therapist, you'll evaluate and treat patients directly within the facility, helping them improve their independence, safety, and ability to perform everyday activities.
You'll maintain clinical autonomy while collaborating with facility staff, physicians, caregivers, and our clinical team.
Responsibilities include:
- Perform occupational therapy evaluations and develop individualized plans of care.
- Provide one-on-one occupational therapy treatments.
- Help patients improve independence with activities of daily living.
- Address upper-extremity strength, coordination, range of motion, cognition, safety, and functional performance.
- Provide therapeutic activities and exercises based on each patient's goals.
- Recommend adaptive equipment and strategies when appropriate.
- Educate patients and caregivers on adaptive techniques and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, facility staff, and members of the interdisciplinary care team.
Compensation:
PRN 1099 – Minimum 5 Patients:

Qualifications:
- Active Occupational Therapist license in New York required.
- Graduate of an accredited Occupational Therapy program.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to collaborate effectively with facility staff and the interdisciplinary care team.
- Geriatric or facility-based therapy experience is preferred but not required.
