Occupational Therapist (OT)
Stern at Home Therapy is hiring Full-Time, Part-Time, and PRN Occupational Therapists (OTs) to provide one-on-one care to patients throughout Manhattan.
We currently have cases available on both the East Side and West Side, allowing therapists to build a caseload in the parts of Manhattan that best fit their schedule and commute.
This is an excellent opportunity for Occupational Therapists looking for flexible scheduling, strong earning potential, clinical autonomy, and individualized patient care without the double-booking and traditional productivity quotas common in other therapy settings.
What You'll Do:
As an Occupational Therapist with Stern, you'll evaluate and treat a primarily geriatric patient population in their homes while developing individualized plans of care focused on improving independence, safety, and quality of life.
Our administrative team supports referrals, scheduling, authorizations, and patient coordination so you can focus on delivering excellent patient care.
Responsibilities Include:
- Perform occupational therapy evaluations and develop individualized plans of care.
- Provide one-on-one occupational therapy treatments in patients' homes.
- Help patients improve independence with activities of daily living.
- Address strength, range of motion, coordination, cognition, safety, and functional performance.
- Provide therapeutic activities and exercises based on each patient's individual goals.
- Recommend adaptive equipment and home modifications when appropriate.
- Educate patients and caregivers on adaptive strategies, exercises, and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and interdisciplinary care team members.
Compensation:
Full-Time – Minimum 16 Patients:
W-2 Salary Option:
- $92,000–$110,000 annually.
- Salary may increase based on patient volume.
W-2 Tiered Per-Visit Option:
- $55–$70 per follow-up visit.
- $75 per 4-unit evaluation.
- Your per-visit rate increases as your monthly patient volume grows.
1099 Tiered Per-Visit Option:
- $70–$80 per follow-up visit.
- $85 per evaluation.
- Annual earning potential up to $145,000.
- Your per-visit rate increases as your monthly patient volume grows.
Part-Time – Minimum 10 Patients:
Part-Time W-2:
- $65 per follow-up visit.
- $75 per 4-unit evaluation.
Part-Time 1099:
- $72 per follow-up visit.
- $85 per evaluation.
Part-time compensation is paid at a fixed per-visit rate and does not use a tiered volume model.
PRN 1099 – Minimum 5 Patients:
- $72 per follow-up visit.
- $85 per evaluation.
- PRN positions are 1099 independent contractor roles only.
Full-Time W-2 Benefits:
- Health insurance and/or medical stipend.
- 401(k) with annual match – starts at 1.5% and can increase up to 7%.
- Paid Time Off – 2 weeks after 6 months and a third week after 9 months.

Qualifications:
- Active and registered New York State Occupational Therapist license required.
- Graduate of an accredited Occupational Therapy program.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to work independently in a home-based environment.
- Home health or geriatric experience preferred but not required.
