Occupational Therapist (OT) – Home Health | North Buffalo & Cheektowaga, NY | Part-Time
Stern at Home Therapy is hiring a Part-Time Occupational Therapist (OT) to provide individualized, one-on-one care to patients throughout North Buffalo, Cheektowaga, and surrounding communities.
This is an excellent opportunity for an Occupational Therapist looking for a consistent part-time caseload with flexible scheduling and strong clinical autonomy.
With a minimum commitment of 10 patients, therapists can build a geographically concentrated caseload across the North Buffalo and Cheektowaga area while receiving dedicated administrative support for referrals, scheduling, authorizations, and patient coordination.
What You'll Do:
As an Occupational Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes, helping patients improve or maintain independence with daily activities and functional tasks.
Responsibilities include:
- Perform occupational therapy evaluations and develop individualized plans of care.
- Provide one-on-one occupational therapy treatments in patients' homes.
- Help patients improve independence with activities of daily living.
- Address upper-extremity strength, range of motion, coordination, cognition, safety, and overall functional performance.
- Provide therapeutic activities and exercises based on individual patient goals.
- Recommend adaptive equipment, assistive devices, and home modifications when appropriate.
- Educate patients and caregivers on adaptive strategies, exercises, and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and interdisciplinary care team members.
Compensation:
- Part-Time – Minimum 10 Patients.
- Part-Time W-2.
- $55 per follow-up visit.
- $65 per 4-unit evaluation.
- Fixed per-visit compensation with no tiered volume model.
Part-Time 1099:
- $62 per follow-up visit.
- $75 per evaluation.
- Fixed per-visit compensation with no tiered volume model.
Part-Time W-2 Benefits

Qualifications:
- Active Occupational Therapist license in New York required.
- Graduate of an accredited Occupational Therapy program.
- Reliable transportation required.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to work independently in a home-based environment.
- Home health or geriatric experience preferred but not required.
