Occupational Therapist (OT) – Mobile Outpatient | Niagara County, NY | Part-Time

Stern at Home Therapy is hiring a Part-Time Occupational Therapist (OT) to provide individualized, one-on-one care to patients throughout Niagara County, NY.

This is an excellent opportunity for an Occupational Therapist looking for a consistent part-time caseload, flexible scheduling, and strong clinical autonomy while treating patients one-on-one in their homes.

With a minimum commitment of 10 patients, therapists can build a geographically concentrated caseload within Niagara County while receiving dedicated administrative support for referrals, scheduling, authorizations, and patient coordination.

What You'll Do:

As an Occupational Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes, helping patients improve or maintain independence with daily activities and functional tasks.

Responsibilities Include:

Perform occupational therapy evaluations and develop individualized plans of care.
Provide one-on-one occupational therapy treatments in patients' homes.
Help patients improve independence with activities of daily living.
Address upper-extremity strength, range of motion, coordination, cognition, safety, and overall functional performance.
Provide therapeutic activities and exercises based on individual patient goals.
Recommend adaptive equipment, assistive devices, and home modifications when appropriate.
Educate patients and caregivers on adaptive strategies, exercises, and safety.
Monitor patient progress and modify treatment plans as clinically appropriate.
Complete accurate and timely clinical documentation.
Communicate with physicians, caregivers, families, and interdisciplinary care team members.

Compensation:

Part-Time – Minimum 10 Patients

Part-Time W-2:

$55 per follow-up visit.
$65 per 4-unit evaluation.
Fixed per-visit compensation with no tiered volume model.

Part-Time 1099:

$62 per follow-up visit.
$75 per evaluation.
Fixed per-visit compensation with no tiered volume model.

Part-Time W-2 Benefits:

Prorated commuter stipend based on patient volume, up to $175 per month for clinicians seeing 10 patients.

1099 independent contractors are not eligible for W-2 employee benefits.

Qualifications:

Active Occupational Therapist license in New York required.
Graduate of an accredited Occupational Therapy program.
Reliable transportation required.
Strong clinical, communication, documentation, and time-management skills.
Ability to work independently in a home-based environment.
Home health or geriatric experience preferred but not required.
