Physical Therapist (PT) responsibilities and duties:

We allow the therapist creative autonomy for treatment plans as well as scheduling.
Pay visits to patients' homes two to three times a week for treatment.
Record your observations in PT initial evaluation, daily notes and progress notes.
Be responsive to our referral inquires.
Possess sharp assessment skills and the ability to work under pressure with patients of all diagnoses.
Have strong clinical skills, including the ability to assess subtle changes with a patient
Educate and instruct patients in rehabilitative care and activities necessary to promote the patient's health, safety, and independent living.
Constantly monitor and manage patient goals and treatment plans.

Physical Therapist (PT) qualifications and skills:

Compassionate, like one on one care, creative treatment plans, and time management skills.
Minimum of 1-year therapy experience is required.
Adaptable, good communication, great rapport.
Previous home health care experience is preferred.
Current state license as a Physical Therapist.
Current CPR certification.
Must be dependable, resourceful and responsible.
