Responsibilities:
- We allow the therapist creative autonomy for treatment plans as well as scheduling.
- Pay visits to patients' homes two to three times a week for treatment.
- Record your observations in PT initial evaluation, daily notes and progress notes.
- Be responsive to our referral inquires.
- Possess sharp assessment skills and the ability to work under pressure with patients of all diagnoses.
- Have strong clinical skills, including the ability to assess subtle changes with a patient.
- Educate and instruct patients in rehabilitative care and activities necessary to promote the patient's health, safety and independent living.
- Constantly monitor and manage patient goals and treatment plans.
Requirements:
- Compassionate, like one on one care, creative treatment plans, and time management skills.
- Minimum of 1-year therapy experience is required.
- Adaptable, good communication, great rapport.
- Previous home health care experience is preferred.
- Current state license as a Physical Therapist.
- Current CPR certification.
- Must be dependable, resourceful and responsible.
