What You’ll Do:

Provide 1:1 therapy sessions in patients’ homes
Evaluate and treat a primarily geriatric population
Create and implement individualized care plans
Maintain clear and timely documentation
Collaborate with a supportive clinical team

Qualifications:

Licensed PT in the state of GA
Degree in Physical Therapy
