Responsibilities:
  • Evaluate and treat patients in the home setting.
  • Develop individualized plans of care.
  • Provide therapeutic interventions and functional training.
  • Educate patients and caregivers.
  • Complete EMR documentation.
Requirements:
  • Active NY license as a PT.
  • Degree from an accredited PT program.
  • Reliable transportation.
  • Strong communication skills.
