About the Job
Physical Therapist (PT) – PRN / Part-Time / Full-Time | 1:1 Home Care | Total Flexibility

Stern At Home Therapy | Warwick & Monroe, NY

Take Physical Therapy out of the clinic—and into real life. If you're looking for true flexibility, better pay control, and the ability to focus on patient care (not productivity quotas), this is the role you've been waiting for.

THE FLEXIBILITY YOU WANT: Design your schedule. Treat your way.
  • Set your own availability – work as much or as little as you want.
  • 1:1 patient care only – no double-booking, ever.
  • Full clinical autonomy – create and execute your own treatment plans.
  • PRN that actually feels PRN – no pressure, no minimums.

THE EARNINGS & MODEL YOU DESERVE:
  • Per-visit rates range from $55 to $75, depending on experience and insurance needs.
  • Tiered pay model – earn more as you take on more visits:
    • Tier 1 (1–20 visits/week): base rate.
    • Tier 2 (21–32 visits/week): +$5 per visit.
    • Tier 3 (33–40 visits/week): +$5 per visit.
  • Full-time salary range: $92K–$110K, based on experience and insurance needs.
  • Faster onboarding = faster earnings (start within ~2 weeks).

THE CASELOAD YOU NEED Stern Scheduling Assurance Plan. We're confident we can fill your schedule. If we can't:
  • Get paid for up to 2 cancellations per month that we're unable to replace.

THE SUPPORT YOU DESERVE:
  • Dedicated admin team handles scheduling, documentation support, and coordination.
  • You focus on patient care. We handle the rest.
  • New to home care or just want extra confidence before going solo? Shadowing is available to everyone — no questions asked.

PERKS THAT ACTUALLY HELP:
  •  We built this role around what clinicians actually need.

Full-Time:
  • Medical, dental, and vision insurance.
  • PTO & Paid Holidays.
  • $250 commuter stipend.
  • Student Loan Repayment – $125/month (begins after your first year).
  • 401(k) Retirement Plan.
  • Quarterly Wellness Stipend – $125/quarter (begins after your second quarter).
  • Childcare Assistance.
  • Continuing Education Support – $500/year.

Part-Time:
  • Medical Stipend – up to $250/month toward your own premium coverage.
  • $75-125 monthly commuter stipend.
  • Student Loan Repayment – $125/month (begins after your first year)
  • 401(k) Retirement Plan.
  • Quarterly Wellness Stipend – $125/quarter (begins after your second quarter)
  • Continuing Education Support – $250/year.

WHAT YOU'LL NEED:
  • Degree in Physical Therapy.
  • Active NY license.
  • Minimum 1 year of U.S. clinical experience (home care preferred)
  • Current CPR certification.
  • Strong communication and bedside manner.

If you're ready to take control of your schedule, earn more per visit, and deliver meaningful 1:1 care, we'd love to connect. Send your resume to ysmith@sternathometherapy.com or call/text 845-879-2467. We respond within 24 hours.
About Stern At Home Therapy
Here's the revised version with the dashes removed:

About Us
At Stern at Home Therapy, we believe great care starts with taking care of the people who provide it. Our mission is simple: deliver exceptional therapy in the place patients feel safest, home. But we can't do that without building an environment where our therapists feel supported, valued, and empowered to do their best work.

For Our Therapists
You're not just staff here; you're the driving force behind our vision. That's why we invest heavily in the tools, mentorship, and resources you need to grow, whether you're just starting or leading a specialty practice. We celebrate wins, encourage professional development, and make sure you have what you need to focus on what matters most: your patients.

For Our Patients
Every therapy plan we build is custom, because no two recoveries look the same. We focus on real comfort and real progress, not one-size-fits-all treatment.

For Our Partners
Our medical associates and partner organizations count on us for financial reliability and consistent, dependable care delivery. That stability is part of what makes Stern a place people want to build a career.

Our Story
Stern at Home Therapy didn't start with a business plan. It started with listening. In our early years, our founders built outpatient services for seniors, and quickly learned that the biggest barrier to good care wasn't quality. It was getting there. Patients were skipping appointments simply because the commute was too hard.
So we brought care home instead. What began as a solution to a scheduling problem became a movement rooted in genuine relationships, the kind where you become a trusted name in someone's family, not just a provider on a schedule.

Years later, that same philosophy drives everything we do. We've grown, but we haven't forgotten why we started: because convenience and connection shouldn't be trade-offs in healthcare, and neither should having a job you actually love.
