Position Overview:
We are seeking a motivated and licensed Physical Therapist (PT) to join our team full-time, providing care both at our facility in LeRoy, NY, and to patients in the surrounding community. The ideal candidate is clinically versatile, comfortable transitioning between facility and community-based settings, and committed to helping patients achieve their highest level of functional independence. Key Responsibilities:
  • Provide physical therapy services to patients at our LeRoy, NY facility and in community settings in the surrounding area.
  • Evaluate, diagnose, and develop individualized treatment plans addressing mobility, strength, balance, pain management, and functional rehabilitation.
  • Implement and progress treatment plans in accordance with physician orders and patient goals.
  • Maintain accurate, timely clinical documentation in compliance with facility, community care, and regulatory standards.
  • Collaborate with an interdisciplinary team including physicians, speech-language pathologists, occupational therapists, and nursing staff.
  • Provide patient and caregiver education to support recovery and functional independence.
  • Conduct home and community environment assessments to inform treatment planning as needed.
  • Participate in care planning meetings and contribute to quality improvement initiatives.
  • Adhere to all applicable federal, state, and facility compliance standards.

Qualifications:
  • Doctor of Physical Therapy (DPT) from an accredited program is required.
  • Current New York State licensure as a Physical Therapist (required).
  • Experience in a skilled nursing facility, outpatient, or community-based setting preferred.
  • Ability to manage a caseload across both facility and community environments.
  • Reliable transportation and a valid driver's license are required for community visits.
  • Strong clinical documentation skills and familiarity with electronic health records (EHR).
  • Excellent interpersonal, organizational, and communication skills.

Compensation:
  • Competitive salary or tiered pay-per-visit commensurate with experience | W-2 or 1099 Employment
