PRN Physical Therapist (PT) – Home Health | Locust Grove, GA | $72–$85 Per Visit
Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients in their homes in the Locust Grove, GA area.
Earn $72 per follow-up visit and $85 per evaluation while maintaining the flexibility to accept patients based on your availability.
This opportunity is ideal for a licensed Physical Therapist looking to supplement their current income, add flexible patient visits to their schedule, or gain experience in home-based care without the double-booking, productivity quotas, and clinic politics common in traditional outpatient settings.
What You'll Do:
As a PRN Physical Therapist, you will evaluate and treat a primarily geriatric patient population in their homes.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one treatment based on each patient's functional needs and goals.
- Accept and manage patients based on your availability.
- Build and organize your schedule around your accepted caseload.
- Complete accurate and timely clinical documentation.
- Communicate with patients, families, physicians, and members of the clinical team as needed.
- Maintain compliance with professional standards and applicable regulations.

Qualifications:
- Active Physical Therapist (PT) license in the state of Georgia required.
- Reliable transportation required.
- Strong clinical, communication, and time-management skills.
- Ability to work independently and manage a flexible patient caseload.
- Home health experience is preferred but not required.
Compensation:
- $72 per follow-up visit
- $85 per evaluation
Schedule:
- PRN / Per Diem
- Flexible scheduling based on your availability and patient needs.
Location:
Locust Grove, GA and surrounding area.

Stern at Home Therapy didn't start with a business plan. It started with listening. In our early years, our founders built outpatient services for seniors, and quickly learned that the biggest barrier to good care wasn't quality. It was getting there. Patients were skipping appointments simply because the commute was too hard.
So we brought care home instead. What began as a solution to a scheduling problem became a movement rooted in genuine relationships, the kind where you become a trusted name in someone's family, not just a provider on a schedule.
Years later, that same philosophy drives everything we do. We've grown, but we haven't forgotten why we started: because convenience and connection shouldn't be trade-offs in healthcare, and neither should having a job you actually love.
