Physical Therapist (PT) – Assisted Living Facility | Le Roy, NY | Full-Time

Stern at Home Therapy is hiring a Full-Time Physical Therapist (PT) to provide maternity leave coverage at an established Assisted Living Facility (ALF) in Le Roy, NY, beginning in October 2026.

This is an excellent opportunity for a Physical Therapist looking for a full-time, facility-based assignment with an established caseload. All patients are treated at one location, eliminating the driving and travel associated with traditional home health.

The PT will cover a current clinician's maternity leave, maintaining continuity of care for the facility's patients.

What You'll Do:
As the covering Physical Therapist, you'll evaluate and treat patients within the assisted living facility and help ensure continuity of therapy services throughout the maternity leave period.

Responsibilities include:
  • Perform physical therapy evaluations and develop individualized plans of care.
  • Provide one-on-one physical therapy treatments.
  • Help patients improve strength, balance, mobility, endurance, and functional independence.
  • Provide gait training, transfer training, therapeutic exercise, and fall-prevention interventions.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Recommend assistive devices and adaptive equipment when appropriate.
  • Educate patients and caregivers on exercises, mobility strategies, and safety.
  • Complete accurate and timely clinical documentation.
  • Communicate with physicians, caregivers, facility staff, and interdisciplinary care team members.
  • Maintain continuity of care throughout the maternity leave coverage period.

Compensation:
  • Full-Time Temporary Coverage.
  • W-2: $40–$45 per visit.
  • 1099: $50 per visit.
  • Established full-time facility caseload.

Qualifications:
  • Active Physical Therapist license in New York required.
  • Graduate of an accredited Physical Therapy program.
  • Ability to commit to full-time coverage during the maternity leave period.
  • Strong clinical, communication, documentation, and time-management skills.
  • Ability to collaborate effectively with facility staff and the interdisciplinary care team.
  • Geriatric, assisted living, or facility-based therapy experience preferred but not required.
