Physical Therapist (PT) – Home Health | Edison, NJ | PRN

Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients throughout Edison, NJ, and surrounding communities.

This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule, maintain a small caseload, or add flexible home health work while maintaining control over when and where they see patients.

With a minimum commitment of just 5 patients, our PRN option provides flexibility without the double-booking and traditional productivity requirements common in other therapy settings.

What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes.

You'll maintain clinical autonomy over evaluations and treatment plans while our administrative team assists with referrals, scheduling, authorizations, and patient coordination.

Responsibilities Include:
  • Perform physical therapy evaluations and develop individualized plans of care.
  • Provide one-on-one physical therapy treatments in patients' homes.
  • Help patients improve strength, balance, mobility, endurance, and functional independence.
  • Provide gait training, transfer training, therapeutic exercise, and fall-prevention strategies.
  • Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
  • Educate patients and caregivers on exercises, mobility strategies, and safety.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Complete accurate and timely clinical documentation.
  • Communicate with physicians, caregivers, families, and interdisciplinary care team members.

Compensation:
PRN 1099 – Minimum 5 Patients:
  • $72 per follow-up visit.
  • $85 per evaluation.
  • Fixed per-visit compensation with no tiered volume model.
  • PRN positions are 1099 independent contractor roles only.

Qualifications:
  • Active Physical Therapist license in New Jersey required.
  • Graduate of an accredited Physical Therapy program.
  • Reliable transportation required.
  • Strong clinical, communication, documentation, and time-management skills.
  • Ability to work independently in a home-based environment.
  • Home health or geriatric experience preferred but not required.
