You'll bring rehabilitation directly to patients in the comfort of their homes, with a supportive clinical team behind you every step of the way.
Compensation — Full-Time:
Earn $58,000–$105,300 depending on your employment type and patient volume.

- W2 Salary: $58,000–$75,000/year (may increase based on patient volume).
- W2 Tiered: $35–$50 per visit, rate increases with weekly volume.
- 1099 Tiered: $50–$60 per visit, with earning potential up to $105,300/year.
Compensation — Part-Time:
- W2: $35–$50 per visit.
- 1099: $50–$60 per visit.

What you'll do:
- Deliver skilled PT treatments in patients' homes across the Monroe area.
- Work under the supervision of a Physical Therapist (including remote supervision).
- Build relationships with a consistent patient caseload.
- Complete documentation using our streamlined process.
- Coordinate with patients, families, and the broader care team.
What we're looking for:
- Active New York PTA license.
- Associate degree from an accredited PTA program.
- Comfortable working independently in a field-based setting.
- Strong communication and organizational skills.
- Valid driver's license and reliable transportation.
- Passion for delivering patient-centered care.
