What You’ll Do:

Provide 1:1 therapy sessions in patients’ homes.
Evaluate and treat a primarily geriatric population.
Create and implement individualized care plans.
Maintain clear and timely documentation.
Collaborate with a supportive clinical team.

Qualifications:

Licensed OT in the state of NY
Degree in Physical Therapy
Strong communication and clinical skills
Home health experience (preferred, not required)
